Office Policies

  • We require 24-hour notice for appointment cancellations
  • Co-pays and deductibles are due at the time of service
  • We ask that you arrive 15 minutes before your scheduled appointment
  • Please bring your insurance card and photo ID to every visit

Medical Records

To request a copy of your medical records, please contact our office at 972-475-9505. Please allow 5-7 business days for processing. Records can be sent electronically via the patient portal or by secure fax.

Prescription Refills

For prescription refills, please contact your pharmacy directly and they will fax a refill request to our office. You can also request refills through the Aprima Patient Portal. Please allow 48-72 hours for processing.

After-Hours Care

For urgent medical concerns outside of office hours, please call our main number at 972-475-9505 to reach our answering service. For emergencies, please call 911 or go to your nearest emergency room.
